Premium+ = $10,000 staked COCA ($15,000), 6% cashback on the first $5,000 monthly, then 1%, three subscriptions included, and 5% unlimited cashback

Costs

There are no monthly or annual fees for the virtual card; the physical card costs $5 for the Starter, Standard, and Standard+ plans, and is free for the others.
