Fail-Safe: Definition

A fail-safe in engineering is a design feature or practice that in the event of a specific type of failure, inherently responds in a way that will cause no or minimal harm to other equipment, the environment or to people.

Unlike inherent safety to a particular hazard, a system being "fail-safe" does not mean that failure is impossible or improbable, but rather that the system's design prevents or mitigates unsafe consequences of the system's failure.

That is, if and when a "fail-safe" system "fails", it is "safe" or at least no less safe than when it was operating correctly.

There have been many movies with the theme of a fail-safe gone awry (e.g., "2001: A Space Odyssey", "War Games", "Terminator", etc.).

Here's a classic one - "Fail-Safe"

The 1964 movie "Fail Safe", directed by sidney Lumet and starring Henry Fonda, Dan O'Herlihy, Walter Matthau, Frank Overton, Larry Hagman, Fritz Weaver, Dom DeLuise and Sorrell Booke, focuses on Cold War tensions between the Soviet Union and the United States.

In the movie, the Fail-Safe for inadvertent ICBM launches proves inadequate and results in an accidental thermonuclear first strike.

A live, televised version of "Fail Safe" (produced in black and white) and starring George Clooney, Richard Dreyfuss and Noah Wyle was aired on CBS in 2000.

Believing that school was too boring, he dropped out of High School early; only to earn an AA, BS and MBA in less than 4 years much later in life – while working full-time as a Navy/Marine Corps Medic.

In spite of a fear of heights and deep water, he performed high altitude, free-fall parachute jumps and hazardous diving ops in deep, open ocean water.
