This story tells of a father in California, struggling against "gender-confirming care." He knows that his son is a boy, but is up against California pushing his son to be a girl. His son, at the time of this episode, is just shy of being 5. That a child is even being considered for gender changing is considered controversial, and the concerns of the Father are apparently not being taken into consideration.
